Our Hospice Care Team Roles

Hospice Physician

Each patient in our care is assigned a hospice physician who oversees their medical treatment, monitors the progression of their illness, and coordinates with other team members to manage symptoms and medications effectively. These physicians work closely with the patient’s own doctor to integrate personal preferences into the care plan.

Hospice Nurse

Our hospice nurses are experts in pain and symptom management, providing direct patient care with sensitivity and expertise. They are also skilled communicators, offering comfort and guidance to families, teaching them how to care for their loved one at home.

Hospice Aide

Hospice aides, who are certified nursing assistants, support patients with personal care needs such as bathing, dressing, and daily hygiene. They also assist with light household tasks to alleviate the load on family caregivers.

Hospice Social Worker

Social workers in our hospice team offer essential emotional and psychosocial support, coordinating logistical aspects of care such as insurance processing and funeral planning. They are a compassionate presence for patients and families, helping them navigate complex emotional and practical challenges.

Hospice Chaplain

Our chaplains provide spiritual support that respects a wide array of beliefs and religious traditions, ensuring that patients and families receive comfort aligned with their values. They work cooperatively with patients' own spiritual advisors to provide a harmonious and holistic approach to end-of-life spiritual care.

Hospice Volunteer

Trained volunteers provide companionship and practical help, whether it's through visiting, assisting with patient care, or helping in our offices. Their contributions are invaluable, offering patients and families additional support and relief during difficult times.

Bereavement Specialist

Our bereavement specialists support families before and after the loss of a loved one, providing grief counseling, support groups, and education for up to 13 months following a death. This ongoing support is crucial in helping families navigate their journey through grief.

Regular Visits and 24-Hour Support

Our team members make regular visits to patients, ensuring that they are comfortable and their needs are met across all dimensions of care—physical, emotional, and spiritual. On average, team members visit patients 5-6 times per week. For any urgent needs outside of regular visits, our 24-hour telephone support line provides immediate access to hospice clinicians who can answer questions and dispatch assistance if necessary.
